To serve CAA Members better, we have made the CAA Guaranteed Life Insurance Plan Application Form available for download. If you are resident in Canada, between the ages of 50 and 75 and wish to take advantage of our offer to gain the peace of mind that CAA Guaranteed Life Insurance can bring, simply click on the link below to download the Application Form (in .PDF format) which you can print, complete and mail to Manulife Financial.

Included with the Application Form is a table of monthly premiums for coverage in the amounts of $2,500 (1 unit), $5,000 (2 units), $10,000 (4 units), $15,000 (6 units) or $25,000 (10 units). If you wish to apply for CAA Guaranteed Life Insurance coverage in some other multiple of $2,500 (to a maximum of $25,000), simply multiply the monthly premium for $2,500 the appropriate number of times. Or, for your complete convenience, let us calculate your monthly premium for you.
