To better serve you, we have made the forms for some of our most common service requests available for downloading. If you wish to make one of the following changes to your coverage, simply click on the appropriate link below to download a form (in .PDF format) which you can print, fill out and mail to Manulife Financial:

	I wish to change the name under which I am covered because I have changed my legal name.
	I wish to apply for Term Life coverage using a printer-ready mail-in application (If you prefer, you can apply online quickly and securely.)
	I wish to apply for Critical Illness Insurance using a printer-ready mail-in application. (If you prefer, you can apply online quickly and securely.)
	I wish to apply to resume my Health, Dental Care, Disability Income Replacement, or Business Expense Insurance coverage using a printer-ready mail-in application. (I had been covered for at least 2 consecutive years and as recently as 10 years ago under the Engineers Canada sponsored plan, I can furnish proof of continuous coverage under my employer's plan in the interim, I am applying within 90 days of termination of my employer's plan and I am under age 55.)
	I wish to apply for lower Non-Smoker rates because I have not smoked for the last 12 months or more.
	I wish to change my beneficiary (and I understand that if my current beneficiary is irrevocable or preferred, I will need my beneficiary's consent to make this change).
	I wish to make a claim for Extended Health benefits using a printer-ready mail-in form.
