"Rewards for Life®" Plans featuring an
opportunity to collect AIR MILES® reward miles

Here we have answers to some of the most frequently asked questions about the "Rewards for Life®" Plans. How can I collect AIR MILES® reward miles?
If you choose to pay your insurance premiums monthly, reward miles can be credited to your AIR MILES® Collector Account each month. For details on the number of reward miles you could collect, click here. The number of reward miles you collect for each Plan does not increase with the amount of coverage you choose.

What if I change my mind after applying for coverage?
Once you receive your Policy, examine it carefully. If you are not completely satisfied, simply return it to us within 30 days, indicating that you would like to have your coverage cancelled and your premiums refunded in full. We'll be happy to do this for you — no questions asked.
When will my coverage begin?
In the case of the "Rewards for Life®" Term Life Plan, the "Rewards for Life®" Guaranteed Life Plan or the "Rewards for Life®" Critical Illness Plan, so long as you are eligible to apply, your coverage will begin on the date when Manulife Financial receives your completed application and your premium payment, if on that date you are insurable in accordance with our underwriting rules. In the case of the "Rewards for Life®" Hospital Cash Plan, your coverage will begin on the first of the month following the month when Manulife Financial receives your completed application and your premium payment. If, when reviewing your application, we find that you do not qualify for coverage, we will notify you immediately and return your premium payment in full.
What if I smoke occasionally?
You are entitled to apply for Non-Smoker rates if, as of the date your application is received by Manulife Financial, you have not smoked any cigarettes in the previous 12 months. (Non-Smoker rates are available under the "Rewards for Life®" Term Life Plan, the "Rewards for Life®" Critical Illness Plan and the "Rewards for Life®" Guaranteed Life Plan. To qualify for Non-Smoker rates, you must also meet our health standards.) If you apply for additional coverage, your smoking status as of the date we receive your application will apply to your coverage in its entirety.
What if I quit smoking after I become covered?
Notify us after 12 consecutive months during which you have not smoked cigarettes. We will mail you a Non-Smoker Application to complete and return to us. You can also download a Non-Smoker Application here. If your Non-Smoker Application is approved (and you meet our health standards and are covered under the "Rewards for Life®" Term Life Plan, the "Rewards for Life®" Critical Illness Plan or the "Rewards for Life®" Guaranteed Life Plan), your lower Non-Smoker rates will begin on the first of the month following the date we received your Non-Smoker Application.
I have received my Premium Notice. What do I need to do?
If your payment method of choice is either by credit card or Pre-Authorized Collection (PAC), and the credit card or account number shown on your Premium Notice is correct, you don't need to do anything. We will charge your premium payment to your account in the month in which your premium is due, and apply it to your coverage. If your Payment Method is by cheque, send a cheque for the full annual premium amount to us by the anniversary of the Effective Date of your Policy.
How do I increase the amount of my "Rewards for Life®" Term Life Plan coverage?
Simply complete another application for the additional coverage amount, then send it to us. If you meet the eligibility and underwriting requirements and pay the first premium, we will increase your coverage. For your convenience, you can apply online or, if you prefer, you can download a printer-ready application.
How do I extend coverage to my dependents under my "Rewards for Life®" Hospital Cash Plan coverage?
To extend "Rewards for Life®" Hospital Cash Plan coverage to your spouse and/or eligible children, simply complete another application for family coverage, then send it to us. For your convenience, you can apply online, or you can download a printer-ready application form.
How do I increase the amount of my "Rewards for Life®" Critical Illness Plan coverage?
Simply complete another application for the additional coverage amount, then send it to us. If you meet the eligibility and underwriting requirements and pay the first premium, we will increase your coverage. For your convenience, you can apply online, or you can download a printer-ready application form that you can mail in.
How do I increase the amount of my "Rewards for Life®" Guaranteed Life Plan coverage?
Simply complete another application for the additional coverage amount, then send it to us. If you meet the eligibility requirements and pay the first premium, we will increase your coverage. For your convenience, you can submit an online request for an application that you can mail in.
How do I change the way I pay my premiums?
You can choose to pay your premiums by cheque, by credit card, or directly from your chequing account by Pre-Authorized Collection (PAC). You can change to one of these Payment Methods at any time by completing and returning the form on the back of your Statement. How do I change my address?

How do I change my "Rewards for Life®" Plan life insurance beneficiary?
You can change the individual or institution you choose to receive your life insurance benefit by completing and signing a Change of Beneficiary form and mailing it to us. How can Best Doctors® help me if I have a "Rewards for Life®" Critical Illness Plan claim?
Once Manulife Financial receives your Critical Illness Plan claim, you will be given access to the Best Doctors® patient referral network. Best Doctors® will offer you the following services:
- Best Doctors InterConsultation™ service provides verification and confirmation of your diagnosis, thorough review of your records by a leading specialist, and collaboration with your physician to determine your best treatment options.
- Best Doctors FindBestDoc™ service includes nurse-assisted research to identify and recommend the best doctors in Canada or abroad to evaluate and treat your condition.
- Best Doctors FindBestCare™ service assists you in accessing care at the top U.S. hospitals for your condition, and provides assistance with the costs of travel and treatment.
If you decide to use this service, you will be responsible for authorizing your physician to send your medical records to Best Doctors® and for paying the costs of any treatment you receive. Note that Manulife Financial does not guarantee the availability of this service and reserves the right to withdraw the service at any time.
Are there any limitations or exclusions to the "Rewards for Life®" Plan life insurance coverage?
No benefit is payable for suicide within the first two years of coverage, other than a refund of payments paid to that date. It is important that you read your Policy when you get it to understand exactly what you are covered for.
Are there any limitations or exclusions to the "Rewards for Life®" Hospital Cash Plan coverage?
This insurance does not cover any period of hospitalization caused by or the result of a pre-existing condition, unless the insured person has been free of treatment for that condition for 12 consecutive months. (A pre-existing condition means a condition, illness, injury or symptom, diagnosed or not, for which treatment or advice was received in the 12 months before the insurance was in force. Treatment or advice means consultation, care, service, diagnostic measures or prescribed drugs provided by a licensed medical practitioner.) This insurance also does not cover any loss or Period of Hospitalization directly or indirectly caused or contributed to by one or more of the following:
- Hospitalization resulting from an accident that occurred before the insured person’s Effective Date of coverage;
- An injury that has no visible wound or contusion except for an injury caused by involuntary asphyxiation or involuntary ingestion of a foreign object that is revealed by an x-ray;
- Intoxication – any event or illness directly or indirectly related to the ingestion of alcohol while the concentration of alcohol in the insured person’s blood exceeds 80 milligrams of alcohol in 100 milliliters of blood;
- Drugs or poison – any voluntary inhalation of gas or ingestion of poison or toxic substances;
- Any voluntary inhalation or ingestion of non-toxic substances, drugs, sedatives or narcotics, whether illicit or prescribed, in such quantity that they become toxic;
- Drug or alcohol addiction – treatment for drug addiction, drug abuse or alcoholism;
- High risk activities – including, but not limited to, participating in a contest of speed, scuba-diving, skydiving, parachuting, hang-gliding, rock or mountain climbing, bungee jumping, or a flight accident except when travelling as a passenger on a commercially licensed airline;
- Mental or nervous disorder – neurosis, psychoneurosis, psychopathy, psychosis, mental or emotional disorder or disease of any kind;
- Uncomplicated pregnancy or childbirth;
- Cosmetic surgery – cosmetic treatment or cosmetic surgery, except when necessary due to accidental bodily injury;
- Criminal offence – from or while committing or attempting to commit a criminal offense, or committing or provoking an assault;
- Suicide – suicide or attempted suicide, self-inflicted injury while sane or insane;
- War or insurrection – declared or undeclared war, or any act of war, riot, terrorism or insurrection, or service in the armed forces of any country or international organization.
It is important that you read your Policy when you get it to understand exactly what you are covered for.
Are there any limitations or exclusions to the "Rewards for Life®" Critical Illness Plan coverage?
No benefit is payable under the "Rewards for Life®" Critical Illness Plan if we determine that:
- at any time before, the Effective Date shown on page 1 of the Policy or the latest reinstatement date of the Policy, the Insured had any type of cancer, whether it is a covered condition or not; or any symptoms or medical problems and such symptoms or problems initiated any investigations that led to a diagnosis of any type of cancer; or
- within 90 days after the Effective Date shown on page 1 of the Policy or the latest reinstatement date of the Policy (the “qualifying period”), the Insured had any type of cancer, whether it is a covered condition or not; or any symptoms or medical problems and such symptoms or problems initiated any investigations that led to a diagnosis of any type of cancer.
That determination will be based on conditions specifically identified in, or that can reasonably be inferred to have existed at that time from, the Insured's application, a related declaration of health, or other information that the Insured authorizes or makes available to us.
Furthermore, benefit is payable under the "Rewards for Life®" Critical Illness Plan if the Insured, while sane or insane, suffers a Covered Condition as a direct or indirect result of any of the following:
- Any illness or injury other than those that meet the definition of Covered Conditions in this Policy;
- Medical or surgical treatment or complications from such treatment;
- Infection;
- Experimental services or treatments or use of new products or new treatments that are not approved for use in Canada or that are being used for a research project;
- Inhalation of gas; absorption or ingestion of poison; use of drugs or alcohol; or use of medication other than as prescribed;
- Intentional self-inflicted injuries;
- Committing or attempting to commit a criminal offence;
- Any act of war, whether declared or not, hostile or warlike action in a time of peace or war, whether initiated by a local government, foreign government or foreign group, civil unrest, insurrection, rebellion, civil war, any act of terrorism including but not limited to, bioterrorism; or the use of force or violence and/or the threat thereof, of any person or group(s) of persons, whether acting alone or on behalf of or in connection with any organization(s) or government(s), committed for political, religious, ideological or similar purposes including the intention to influence any government and/or to put the public, or any section of the public, in fear;
- Operation, or care or control of a motor vehicle, vessel, aircraft or railway equipment, whether or not in motion, while the quantity of alcohol in the Insured's blood exceeds 80 milligrams of alcohol in 100 milliliters of blood

How are the "Rewards for Life®" Plan life insurance premium payments calculated?
Your payments are based on your gender, attained age and smoking status at the time you apply. Term Life payments increase with age, but are guaranteed not to increase during the first five years of your coverage. Guaranteed Life premiums are guaranteed never to increase for as long as you maintain your membership under the Plan and, starting at age 100, the coverage is fully paid-up with no further payments due to continue coverage.
How are the "Rewards for Life®" Critical Illness Plan premium payments calculated?
Your payments are based on your gender, attained age and smoking status. Payments increase at renewal dates based on your age on that date.
How are benefits paid under the "Rewards for Life®" Term Life Plan?
For death occurring in the first two years of the Policy due to suicide, the death benefit is a return of premiums since the Effective Date, without interest. Otherwise, subject to the Policy terms and conditions, the death benefit is the face amount, less any Living Benefit paid.
How are benefits paid under the "Rewards for Life®" Hospital Cash Plan?
In the case of the Collector (and Spouse, if also insured) this Plan pays $175 per day of hospitalization if the insured is a Non-Smoker or $140 per day of hospitalization if the insured is a smoker. To receive Non-Smoker benefits of $175 a day, the insured must not have smoked cigarettes in the 12 months immediately prior to the time of claim. In the case of children, if also insured, this Plan pays $100 per day of hospitalization. All benefits are payable from the 1st day for an accidental injury, and payable from the 1st day for sickness if the insured is continuously hospitalized as an in-patient, and billed, for at least 3 full days. All benefits are payable to a maximum of 365 days per injury or sickness.
How are benefits paid under the "Rewards for Life®" Critical Illness Plan?
The benefit is the face amount, payable after Manulife Financial receives satisfactory proof that you were diagnosed with one of the following Covered Conditions for the first time in your life, while your "Rewards for Life®" Critical Illness coverage was in force, and that you survived the Covered Condition for at least 30 days. Covered Conditions include coronary artery bypass surgery, or the first diagnosis of a heart attack, stroke or life-threatening cancer. You get this money tax-free, in a single cheque made out to you, to spend as you wish.
How are benefits paid under the "Rewards for Life®" Guaranteed Life Plan?
For death occurring in the first two years of the Policy:
- If death is due to non-accidental causes and is not due to suicide, the death benefit is a return of premiums paid since the Effective Date, accrued with interest at 10% per annum compounded annually.
- If death is due to suicide, the death benefit is a return of premiums since the Effective Date, without interest.
- If death is due to accidental causes, the death benefit is five times the face amount.
For death occurring after the first two Policy years:
- If death is due to non-accidental causes at any age, or accidental causes at age 85 or older, the death benefit is the face amount.
- If death is due to accidental causes before the age 85, the death benefit is five times the face amount.
How long will I be covered under the "Rewards for Life®" Term Life Plan?
As long as your payments are made when due, you may renew your coverage until you reach age 70, when coverage ends.
How long will I be covered under the "Rewards for Life®" Hospital Cash Plan?
As long as your payments are made when due, you may renew your coverage until you reach age 65. Coverage ends for all individuals insured under your "Rewards for Life®" Hospital Cash Policy when you reach age 65.
How long will I be covered under the "Rewards for Life®" Critical Illness Plan?
As long as your payments are made when due, you may renew your coverage until you reach age 70. Coverage will end on the date of your 70th birthday or on the date we pay the Critical Illness Benefit to you, whichever comes first.
How long will I be covered under the "Rewards for Life®" Guaranteed Life Plan?
As long as your payments are made when due, you may renew your coverage until you reach age 100, when the coverage is considered fully paid-up, and continues for life with no further payments due.
How can I cancel my coverage?
If you ever wish to cancel your coverage, all you have to do is mail us a written notification bearing your signature. Your coverage will end on the Premium Due date following the date on which we receive your cancellation request. If you mail us your written and signed cancellation request within 30 days of first receiving your Policy, we will refund your premiums in full. In either case, please be aware that if you later choose to rejoin the plan, you must once again complete an application and meet the eligibility qualifications and underwriting requirements of any new applicant.
