- be under Age 36; and
- apply no later than 90 days after becoming a Member of the Institute of Chartered Accountants of Ontario, New Brunswick, Prince Edward Island, Newfoundland and Labrador, Nova Scotia or Bermuda.
If you are applying for Personal Accident Insurance, you must also be covered by or applying for Member Term Life Insurance or Member Income Protection Insurance. |
Term Life Insurance & FIO coverage for Members |
To apply for Member Term Life Insurance, you must be:- a Member of one of the Institutes of Chartered Accountants of Ontario, New Brunswick, Prince Edward Island, Newfoundland and Labrador, Nova Scotia or Bermuda;
- under Age 65; and
- resident in Canada or Bermuda.
The Future Insurability Option (FIO) may only be applied for at the time of application for Member Term Life Insurance. FIO applicants must be between the ages of 18 and 50. |

To apply for Term Life Insurance for your Spouse or common-law Spouse (who must be under Age 65), you must be:- a Member of one of the Institutes of Chartered Accountants of Ontario, New Brunswick, Prince Edward Island, Newfoundland and Labrador, Nova Scotia or Bermuda; and
- resident in Canada or Bermuda.

To apply for Member Personal Accident Insurance, you must be:- covered by or applying for Member Term Life Insurance or Member Income Protection Insurance;
- a Member of one of the Institutes of Chartered Accountants of Ontario, New Brunswick, Prince Edward Island, Newfoundland and Labrador, Nova Scotia or Bermuda;
- under Age 65; and
- resident in Canada or Bermuda.

To apply for the Child Life & Accident Insurance rider on Member or Spouse Term Life Insurance you have or are applying for, your children must be:- unmarried;
- dependent on the Member for support; and
- under Age 21 (or under Age 25 if in full-time attendance at school or university).

| Critical Illness Insurance for Members |
To apply for Member Critical Illness Insurance, you must be:- a Member of one of the Institutes of Chartered Accountants of Ontario, New Brunswick, Prince Edward Island, Newfoundland and Labrador, Nova Scotia or Bermuda;
- under Age 65;
- resident in Canada or Bermuda; and
- free of these specific pre-existing medical conditions or procedures:
- Active hepatitis, AIDS or AIDS-related disease
- Alcohol abuse in the past five years
- Alzheimer’s disease
- Any heart condition or heart trouble (excluding controlled hypertension)
- All cancer except basal cell skin cancer
- Coronary bypass surgery
- Diabetes
- Heart attack
- Huntington’s chorea
- Kidney disease, other than kidney stones or a history of kidney infection
- Lou Gehrig’s disease (Amyotrophic Lateral Sclerosis, or ALS)
- Major organ transplant recipient
- Multiple sclerosis
- Permanent paralysis (paraplegia, quadriplegia), other than Bell’s Palsy
- Pulmonary fibrosis
- Stroke (cerebrovascular accident)
- Transient Ischemic Attack

- resident in Canada or Bermuda; and
- the owner of (or a partner in)
a Firm. |

To apply for Catastrophic Health Insurance for your Spouse, your Spouse must be:someone to whom you are legally married or with whom you have cohabited in a conjugal relationship for at least 12 consecutive months;over Age 20;under Age 65;resident in Canada; andcovered under a provincial or territorial government health care plan.You must be covered by or applying for Catastrophic Health Insurance to also apply for this coverage for your Spouse. |
