To better serve Insured Members, we offer online forms for some of our most common service requests. To submit your request directly to Manulife Financial for any of the following changes to your coverage, simply check your selections and click the button to continue:

You can also download printer-ready forms, which you can complete and mail to us if you wish to change your name, change your smoking status, change your beneficiary or apply for additional coverage.
